On 3/22/24, Licensing Program Analyst (LPA) Priscilla Zamudio and Licensing Program Manager (LPM) Cynthia Brannon conducted an unannounced case management inspection. LPA and LPM met with Site Supervisor, Luis Figueroa and Program Manager, Laura Mitchell at the above facility. The purpose of today’s inspection was to check the newly installed playground structure, which is located in front of preschool class 2. The facility was previously approved for a playground waiver, and are currently sharing a different playground with other children from the pre school.

During the inspection, LPA and LPM toured the facility inside and outside, took a census and interviewed staff. LPA and LPM observed that a new playground structure was installed appropriately and no hazards were observed. There were no loose or pointed parts. LPM advised site supervisor to ensure proper supervision near a small tunnel and to keep all areas clean, removing any dirt or spider webs on the structure. The playground structure had rubber material to cushion any falls. Director submitted playground specifications.

Previously, there were three large trees to provide shade but they were removed. There was one shade structure on the opposite side away from the play structure. Site Supervisor stated that a shade canopy is scheduled to be installed over the play structure, however, the date is unknown.

The following items will need to be corrected prior to the new play structure being used:

- Shade structure over climbing equipment. During today's inspection, LPA observed that the climbing structure did not receive any shade during inspection. LPM, site supervisor and program manager touched the climbing structure. The structure was too hot for children to use. LPM spoke with Charlotte Miranda, Health and Safety Supervisor over the phone, and a temporary shade structure will be installed. Licensee shall contact LPA to conduct another inspection to ensure shade is in place.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with Site Supervisor, Luis Figueroa.
This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days.
