On 11/15/2021, Licensing Program Analysts (LPAs) Juvenal Moctezuma & Nancy Her conducted an announced Case Management inspection and met with Site Supervisor, Lisa Antaramian. LPAs explained the reason of the inspection and a tour of the center was conducted both inside and outside. The center is located on 3920 N. First St in Fresno, CA. Lisa is requesting to increase her Infant capacity to 26 Infant Children. This program will operate year round from 6:30 AM to 6:00 PM Monday through Friday. This program will operate in Infant classrooms # 1, #2, #3, #7 & #8.

Lisa stated that the parents/authorized representatives bring their children diapers, formula, wipes, and food. The center also offers baby food and formula. Lisa stated that they have a plan in place in case parents forget these items. The bottles are labeled with the child's name & date. LPAs observed a refrigerator in classroom #1 where the bottles can be stored. Lisa is proposing to use classroom #1, #2 & #7 for infants activity play area. Classroom #3 & #8 will be utilized for napping only. Classroom #1 also has a separate area for children's cribs which will be rearranged. Lisa stated that she will set up the classrooms accordingly and LPAs notified her that an additional inspection will be required. There will be a changing table in classroom #1, #2, & #7 next to a sink. LPA reminded Lisa that these sinks could not be used for food prepping.

Lisa stated that the church has a janitor that cleans the classrooms during after hours. The cleaning supplies will be stored underneath the children's sink in each classroom or be kept in locked cabinets. LPAs observed disinfecting solutions & hand sanitizers to be out during the inspection. Lisa removed the items and stored them in areas that are inaccessible to children. The cabinets have a lock in place to prevent children from having access. Staff will utilize the staff bathroom located next to the Site supervisors office which will remain closed and inaccessible to children at all times. Ill children will be isolated in the site supervisors office and use the staff bathroom.

The classrooms were observed to be free of toxins. LPA observed that the furniture and equipment appeared to be in good condition. LPAs observed a bouncer in classroom #1. Lisa stated that it belonged to teacher #1. Teacher #1 removed it and stated that she was taking it home. LPA observed the paint to be chipping from the classroom walls and reminded Lisa to repaint the classroom.

LPA reminded Lisa to not store anything on top of the children's cubbies or tall bookcases that could potentially fall on the children. Lisa understood. LPA observed a working carbon monoxide inside the center. Lisa stated that teachers conduct wellness checks every morning during drop off and take the children's and staffs temperatures before entering the classroom.

Room measurements were taken and reviewed with Lisa. The total inside Infant area measured to an approximate 1,115 square feet which will accommodate 26 Infants. Lisa will amend her application and submit the Increase of capacity packet. There is adequate storage space available for children's belongings in every classroom. Lisa will submit proof that there are enough tables, chairs, & cribs, & cots for the 26 Infant capacity she's requesting. Lisa stated that she will set up classroom #3, #7, & #8.

Toys and equipment appear to be age appropriate. Lisa stated that the center washes the infants bedding/sheets every week on Friday or whenever necessary. There are a total of 6 sinks/hand washing fixtures which will accommodate the newly requested capacity 26 infants. Lisa stated that each infant has their own sippy cup for water which is labeled with their name and used both indoor and outdoor.

The approximate outside square footage for the Infant play area is 990 which will only accommodate 13 infants at a time. Lisa stated that she will stagger the infants into two sessions and understands that no more than 13 infants can be outside at a time. Lisa understood. A waiver will be submitted an pending approval. The infant play area has a fence all around. LPAs observed plenty of age appropriate toys and a shade structure covering the whole play area. LPA reminded applicant to provide adequate supervision at all times.

During the tour of the inspection, LPAs observed staff members on their cellphones while supervising infants that were napping. LPAs reminded Lisa to review safe sleep regulations and notified her that staff should be actively supervising children at all times. Lisa understood.

A fire clearance for 128 children was received and granted by the Fresno Fire Department on 10/20/2021. An amended fire clearance will be required to meet the newly overall requested capacity of 101 children.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. The following items must be completed prior to issuing a license by 12/01/2021.

Infant

  • Repaint classrooms since paint is chipping off.
  • Make cleaning supplies Including hand sanitizer inaccessible to children.
  • Rearrange sleeping area in classroom #1, #3, & #8 to make sure there's enough space for 26 infants to nap - either in a crib or cot.
  • Submit proof of cots that are at least 3/4 inch thick.
  • Submit proof of tight fitting sheets/bedding.
  • Get door knob for staff bathroom in classroom #1.
  • Remove items on top of shelving area in sleeping area - classroom #1 to prevent any injury.
  • Move changing tables to be closer to the sink and not take additional square footage from classrooms.
  • Anchor all cubbies and tall book cases to prevent any injuries.
  • Get Additional cubby space so children's belongings are not on the floor or touching one another.
  • Get amended fire clearance & Additional licensing documents.

Pending a final file review and completion of above items, a recommendation will be made to license the above facility for a capacity of 26 infant children.
