LPA conducted an inspection to measure preschool rooms due to water damage that occurred during 7/2019. Licensee closed facility during construction. LPA took measurements of facility. Director, Molly Bennett accompanied LPA. Approximate inside square footage is 3259 which will accommodate capacity of 93 preschool children. Licensee previously utilized the climbing fenced area and the blacktop. During construction, the fencing around the blacktop area was removed. During today's visit, LPA measured and inspected the two play yards; the climbing structure area and the grass area on site. The grass area is further away from the preschool classrooms than the blacktop. Per director, before going outside to play, the children will use the restrooms; adequate staffing to bring children to the restroom from the outside play area and remain within ratio; adequate staffing for supervision when taking the preschool children to and from the outside grass play area; and staff will utilize a rope that the children can hang on to while staff is walking the children from their classrooms to the grass area play yard. The total outside square footage is 18575, which will accommodate the capacity of 93 preschool children. There are 9 toilets and 9 sinks will accommodate the capacity of 93 preschool children. Licensee's climbing structure has a 360 degree slide. Per director, staff are always stationed by the slide to ensure children do not jump off the slide. Lunch are provided off-site and transported to facility. Licensee provides morning and afternoon snacks. Hours of operation is 7 am to 6 pm, Monday through Friday. CONTINUED ON FOLLOWING PAGE

The following items need to be addressed before licensee can accept preschool children:

1) New outside facility sketch that includes the new play yard.
2) All cabinets need to be anchored.
3) In room 313, need changing pad.
4) In room 313, the metal door frame is warped. There's space between wall and vertical metal piece that children can stick their fingers into.
5) In room 313, cabinets/shelving needs to be installed above the changing table.
6) In room 303, the bathroom door does not close.
7) In room 305, the bathroom door is sticking.
8) In room 307, the door leading from the children's bathroom to staff break room needs to have a lock installed.
9) In the areas around the fall zones of the climbing structure, licensee shall install cushioning that will help prevent children from sustaining a serious injury from falls.
10) In the new outside play yard, there are long bolts. These need to be cut down to two or three threads.
11) There are raised tree roots.
12) Licensee shall have the trees trimmed of their lower branches.
13) The bottom of the chain link fencing has sharp points. The bottom of the fencing should be turned instead of twisted.
14) The wooden horizontal slabs are broken and sharp.
15) Cushioning on the metal poles shall be installed to help prevent children from sustaining an injury when they run into the poles.
16) The canvas shade is ripped.
17) There is spacing between the brick fencing and the metal chain link pole to allow a child to escape.
18) An igloo with disposable cup dispenser and disposable cups for the outside drinking water is required.
19) LPA observed bird droppings on the equipment under the tree. Licensee shall ensure that the equipment is cleaned each time before children come out to play. Licensee can provide a cleaning schedule or move the equipment.
20) Clean the spider webs under the tables.

LPA will conduct a return visit to ensure corrections are made before facility provide care services.

Licensee is aware that a new fire clearance will be needed to inspect the multipurpose room and for the new capacity of 93 children. Licensee shall provide a $25 fee for the change in capacity, an updated application LIC 200A reflecting infant/toddler and preschool capacity, a fire clearance request, updated LIC 610 Emergency Disaster Plan, updated LIC 500, updated facility sketches (inside and outside) and a Board resolution stating authorization of the change and authorizing a person to act on their behalf. These forms are and fee shall be sent to the Fresno Community Care Licensing office by November 11, 2019.

Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit. Exit interview conducted with the director, Molly Bennett. A copy of this report need to be placed in facility file for public review. A Notice of Site Visit was posted on parent board.
