On 10/07/21, Licensing Program Analyst (LPA) Juvenal Moctezuma conducted an unannounced inspection to initiate & conclude the complaint investigation that was received on September 14, 2021. LPA met with Site Supervisor Molly Bennett and discussed the purpose of the inspection and investigation findings. A Tour of the center was conducted both inside and outside and census were taken.

During todays inspection LPA observed 76 preschool age children in care whom were not being taught and/or reminded to wear face coverings as required by California Department of Public Health guidelines. LPA obsrved staff wearing masks & 1 staff wearing a face shield with no mask. During the tour, site supervisor acknowledged that the facility was leaving it up to parent’s to decide whether or not their child should wear a face covering while in care.

Based on the information obtained during todays inspection, there is a preponderance of the evidence to prove that this facility is not following COVID-19 mandates; therefore, the allegation is substantiated.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D). Director was provided a copy of their appeal rights.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
