On November 8, 2021, Licensing Program Analyst (LPA) Brannon, conducted an unannounced Annual Required Inspection for the school age license. LPA met with Center Director, Monica Curry, and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday, 7:00 AM to 6:00 PM. Director verified facility phone number is (559) 431-7330.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Per Center Director, no smoking is allowed on site. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

Furniture and equipment are free of sharp, loose or pointed parts. During inspection, LPA observed a vinyl red ottoman with a tear and foam showing. The tear does not allow for staff to sanitize or clean furniture as required. LPA observed two vinyl beige children size furniture with wear spots. LPA recommended for director and staff to monitor the two pieces of equipment. Staff should be informing director and removing or repair child size furniture as required.

Outdoor activity space provided for school-age children is physically separate from space provided for infant care and child care center children. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. School age playground does not have climbing equipment. The play yard is a grassy area. Licensee is utilizing rubber tires for children to stack, roll and play. During today’s inspection, LPA observed a tractor tire that was propped up against the back fence which allowed water to accumulate and had the Center Director observe the standing water in the tractor tire. Another small tire had cracks. During today’s inspection, director had the water removed from the tractor tire and had the tractor tire moved to a sunny area on the grass. The cracked tire was removed from the outside play area.

All toilets and hand washing facilities are in operating condition. During inspection, LPA observed one stall without toilet paper and brought it to Center Director's attention. Preschool children and school age children share the restroom. There is no waiver on file. Per director, when a school age child needs to use the restroom, staff will take the school age child to the restroom, verify that preschool children are not present before the school age child can use the restroom. This restroom is used by both genders. Only one child at a time can use the restroom. LPA informed director that a waiver for sharing the preschool bathroom is required.

Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. The cook utilizes plastic containers and zip lock bags for open bags of food to assist in keeping the facility free from vermin. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

Center Director, Monica Curry, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

The facility maintains compliance with staff-child ratios required in 101516.5(a)-(d).Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The La Petite center director or fully qualified teacher designated to act in the director’s absence has been reported to the Department.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Licensee is utilizing an electronic sign in/out. All individuals who are authorized to sign a child in/out have their own PIN. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 14 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Menus are posted at least one week in advance where an authorized representative can view them.

This facility provides Incidental Medical Services – IMS. At this time, there are no school age children requiring IMS. Currently, licensee does have an infant program. For future reference, LPA discussed the safe sleep regulations with Center Director, Monica Curry, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Center Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
