On 03/22/2022, Licensing Program Analyst (LPA) Candis Rodriguez conducted an unannounced complaint inspection at facility. LPA met with Director Tasha Avalos, explained purpose of inspection, toured the facility, and took a census.

During the investigation, LPA observed the facility and took photographs. On 02/18/2022, LPA observed two dead cock roaches in corners of the Discovery Preschool classroom floor. LPA observed a live cockroach in a corner of the Preschool classroom floor. LPA did not observe any cock roaches in the Pre-K classroom. LPA did not see any poisons or hazardous materials in any of the preschool classrooms.

LPA obtained records from facility indicating facility has been utilizing pest control for this issue. Records from the past three months indicate an issue with cock roaches and is actively being treated regularly. Director stated facility will continue to monitor the situation. Director stated every evening, a designated staff member walks the facility to ensure facility is clean and any dead cockroaches are cleaned and disposed of.

LPA advised Director to work with pest control to ensure the work completed by pest control is successful, and to inquire of other options the facility may need to consider in order to eliminate the pest issue.

Investigation revealed through interview and observation the facility does have cock roaches. Therefore, the preponderance of evidence standard has been met, and the allegation is found to be Substantiated.



Per the California Code of Regulations, Title 22, Division 12, Chapter 1 the following deficiency was cited during today's inspection (See LIC 9099-D). An exit interview conducted with Tasha Avalos.

Notice of Site Visit to be posted for 30 days.

LPA informed Director Tasha Avalos that this report dated 03/22/2022 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.

Also, LPA informed Director to provide a copy of this licensing report dated 03/22/2022 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Director provided proof of regular pest control service and agreed to continue pest control service to eliminate the cock roach issue. Director stated facility has designated staff members who will conduct a walk-through of the facility every evening to ensure facility is clean and any dead cock roaches are cleaned and disposed of.

Based on observation and interview, facility did not ensure center was free from cock roaches and clean, safe, and sanitary at all times. Cock roach infestations can cause health hazards. This poses an immediate risk to the health, safety, and personal rights of children in care.

Director agreed to work with pest control to find a successful solution that will eliminate this issue. POC to be cleared upon subsequent inspection by POC due date.
