25 | On this date, a Case Management incident inspection was conducted by Licensing Program Analysts (LPAs) Diane Mercado and Robert Gutierrez. LPAs met with Director Martha Rollins to discuss an incident report submitted to the Fresno Regional Office regarding an incident that occurred in the facility on October 24, 2019, where three daycare children were left unsupervised in the children’s restroom located inside the classroom.
Director stated on Thursday 10/24/2019, Teacher #1 and Director were caring for 12 napping children. Director stated that she had to use the restroom and left Teacher #1 caring for the 12 napping children. While using the staff restroom, Director overheard someone using the children's restroom located next to the staff restroom. The Director left the staff restroom and observed three daycare children inside the restroom with no teacher supervision. Child #1 and Child #2 were washing their hands and Child #3 was using the restroom. Director stated Teacher #1 did not witness Child #1, Child #2, or Child #3 get up from their cot to use the restroom.
Director stated that both the staff and children's restrooms are inside the classroom and that Child #1, Child #2, and Child #3 did not leave the building.
During today's inspection, LPAs interviewed staff regarding the unusual incident and confirmed the information reported to Community Care Licensing on Monday 10/28/2019. LPA's also reviewed the child's file and inspected the restroom where the children were left unsupervised. Per Director these children were left unsupervised in the restroom for one minute. Exit interview conducted with Director Martha Rollins.

Appeal rights were provided. This report is to be made available to the public upon request. LIC 9224 Acknowledgement of receipt of licensing reports was provided.
