25 | On 03/12/2024, Licensing Program Analyst (LPA) Miguel Herrera conducted an unannounced case management inspection at the facility. LPA met with Site Supervisor, Trenity Jones who accompanied LPA during a tour of the facility both inside and outside. LPA explained the purpose for the inspection and a census was taken. During the case management visit, facility observations were completed, facility records were reviewed, and interviews were conducted with staff.
Based on records review LPA Herrera observed child #1 (age, 6 years old and 1 month) arrived at 8:00AM on 03/12/2024 and was placed in the Rising Star A classroom with preschool age children ages 4-5 years old. Director Jones stated that staff #1 arrives at 9am and therefore child #1 was placed with preschool age children until 9am. Furthermore, LPA Herrera observed attendance records indicating child #1 continuously commingling with preschool age children. Per Director, child #1 is enrolled in the Rocketeers classroom (school age children, ages 6 to 12 years old). Per director Jones, child #1 will arrive at 8am once staff #1 is present in the school age classroom.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, this deficiency is being cited on the attached LIC 809D.
An exit interview conducted with Director Trenity Jones. A copy of this report and Appeal Rights were provided and discussed with Director Jones. A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days. |
