On 10/12/2021, Licensing Program Analyst (LPA) Juvenal Moctezuma conducted an unannounced Case Management inspection and met with Site Supervisor, Veronica Ward. LPA explained the reason of the inspection and a tour of the center was conducted both inside and outside. The center is located on 3839 N. Hughes Ave in Fresno, CA. The licensee is requesting to increase their School Age capacity to 29 children and switching to the other side of the classroom. This program operates year round from 6:30 AM to 06:00 PM. Veronica stated that the program offers breakfast, lunch & 2 snacks. The food is prepared on site in the kitchen every day and then disbursed to the children. Brenda Arrived shortly after the tour.

The kitchen has a half door and Veronica was made aware that it needs to be closed and locked during operating hours since that's where they store all their cleaning items and hazardous products, knives, and sharp utensils. The staff bathroom is located on the preschool side. LPA reminded Veronica to keep the staff bathroom also closed and inaccessible to children. LPA observed a utility room which is used for storage and another room (craft room) that's used for storage. Both rooms also need to be closed and locked since Veronica stated that children don't have access to them. The site supervisors office is also used for storage.

LPA reminded Veronica to keep the preschool classroom clean, free of clutter, and to not store items on high shelves or yellow school bus toy that can potentially fall on a child. LPA also advised Veronica to anchor any cubbies or tall furniture to prevent from tipping over. Veronica stated that the site supervisors office will also remain closed by a plastic door knob since the staffs personal items like purses get stored there. Ill children will be isolated in the site supervisors office or craft room and will use the staff bathroom when ill.

The classroom was observed to have plenty of toys. Licensee will submit proof that there are enough tables & Chairs to meet the newly requested capacity. LPA observed a working carbon monoxide on site. Room measurements were taken and reviewed with Veronica. The total inside school age area measured to an approximate 898 square feet which will only accommodate 25 school age children. Licensee will amend the application to meet the newly measured capacity.

Toys and equipment appear to be age appropriate. There is a total of 2 toilets and 2 sinks/hand washing fixtures in the bathroom which will accommodate the requested capacity. Veronica stated that children bring their own water bottles from home and are placed in the classroom so children can have water readily available to them. Veronica sated that children take out water bottles when playing outside as well.

The approximate outside square footage measurements were taken and reviewed with Veronica. The total outside school age play area measured to an approximate 3,516 SF which will accommodate a total of 46 children at a time. The outdoor play area gets shared with the preschool children. Veronica stated they will submit a waiver to stagger the outdoor play times and understand that both preschool children and school age children cannot be commingled at any time. Waiver will be pending approval.

The outdoor play area has a small basketball court, a play structure, metal dome that children can use and play, and plenty of other toys. Licensee is using wood & rubber mulch to soften the fall zones. LPA advised Veronica to get more cushion material since it appears there's not enough to cushion the fall zones. LPA observed a cement block on the fall slide zone of the play structure that poses a threat to children's safety. LPA observed 2 mature trees that provide shade & a play structure that needs an age sticker. The playground has a iron fence that goes all around.

A fire clearance for 42 children was received and granted by the Fresno Fire Department on 09/08/2021. An amended fire clearance will be sent out to the fire department to meet the newly requested capacity of 37 children.

During the Inspection, LPA observed that the two staff present were wearing face masks underneath their nose and all school age children in care were not being taught and/or reminded to wear face coverings as required by California Department of Public Health guidelines. Veronica acknowledged that the facility was leaving it up to parent’s to decide whether or not their child should wear a face covering while in care and that it was difficult to try and get children to keep face masks on. LPA notified Veronica that according to the state mandates and guidelines all children over the age of 2 and staff should be wearing face masks. Veronica understood and sated that they will start requiring children to wear face coverings while indoors.

This facility plans to provide Incidental Medical Services – IMS. The following items must be completed prior to issuing a license by 10/26/2021.

Classrooms

  • Get door chimes or door alarm for the entrance since the double doors lead to the parking lot and then main street (West Ave)
  • Get toilet caps since screws are exposed.
  • Make sure all cables are not exposed and get outlet covers.
  • Make sure kitchen, craft room, office, utility room, staff bathroom are locked and inaccessible to children.
  • Move items from the top of the yellow school bus so they cannot fall on top of a child.
  • Paint door trim/cubbies/sign in wooden cabinet or any areas that paint is chipping
  • Get additional tables & chairs to meet requested capacity.
  • Get Amended Fire Clearance & LIC 200A.
  • Play yard
  • Get more cushion material to soften fall zones.
  • Age Sticker for Preschool Playground.
  • Fix chain link fence on the north/west side of the building. There's a hole and fence is loose.
  • Paint Iron fence since rust is showing.

A final file review and completion of above items, a recommendation will be made to license the above facility for a capacity of 25 School age children.
