On 6/26/2019 at 7:45 AM, Licensing Program Analysts (LPAs), Stephanie Navarro and Juvenal Moctezuma, conducted an unannounced annual/random inspection. LPAs were greeted by Staff #1 and began the tour inside/outside. An hour later, Director Sandra Dermer arrived and met with LPAs. LPAs toured the facility, both indoors and outdoors. There are two infant classrooms in operation Room #0 and Room #1. There are no firearms present at this school. All children are under supervision, including visual supervision, of a teacher at all times. Teacher/child ratios are maintained and adequate supervision is being provided during this visit. The facility ensures that each infant is never left unattended, and under the direct visual supervision of a staff person at all times. Disinfectants, cleaning solutions and other dangerous items shall be inaccessible to children. No poisons were observed during today’s inspection. All materials and surfaces accessible to children are toxic free. While in use, infant changing tables are placed within arm's reach of a sink. All floors are clean and safe. The facility has age-appropriate furniture and equipment including, but not limited to, cribs, cots, or mats. The facility has sufficient infant napping equipment. LPAs observed two baby bouncers in the infant sleeping area. The facility has indoor activity space for infants that is physically separate from space used preschool program. LPAs observed in the classroom solid waste storage vessels, including moveable bins, have tight-fitting covers on, are in good repair. Drinking water is available both indoors and outdoors. LPAs observed infant bottles found in the refrigerator that, according to the infant teacher, had been brought by parent(s) are not labeled with a current date. All kitchen, food prep, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 (F) or less. Menus are posted at least one week in advance, where an authorized representative can view them. There is a working fire extinguisher, carbon monoxide, smoke detector, and adequate heating and ventilation for safety and comfort. Fire drills are conducted and documented every six months. LPAs observed playground equipment is in good condition, free of sharp, loose, or pointed parts. LPAs reviewed 10 children files that contained required documents and immunization records.

The facility ensures that each infant has an Infant Needs and Services Plan and an individual feeding plan for each infant. LPAs observed infant room #1 do not have the written infant needs and services plan updated at least quarterly when transitioned from infant room #0. LPA Navarro observed 4 infant plans were not signed by the authorized representative.

Before working in a licensed child care facility, all individuals subject to a criminal record review have a clearance or exemption. Staff records contain appropriate, documentation of education credits. Per records review and interview, all staff have completed the AB1207 Mandated Reporter Training. At least one person trained in CPR and Pediatric first-aid is present when children are at the facility or at off-site activities. This facility’s hours of operation are 7:00 AM - 6:00 PM. Facility provides children a morning snack, lunch, and afternoon snack.

Incidental Medical Services (IMS) policy was discussed. Facility is not providing IMS services at this time. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Facility is administering IMS and IMS Plan is on file. LPA & Director discussed the Community Care Licensing website, Lead Safety, and Mandated Reporter Training. LPA and Site Supervisor discussed new additions to the website that include the new PIN (Provider Information Notification) and information for providers including the Quarterly Update that informs licensees of new legislation and regulations. Please follow these steps go to http://www.cdss.ca.gov/, click on “information and resources” click “Community Care Licensing” Click “quarterly updates” click “Child Care advocates program” and register to PIN.

Per Chapter 1, Division 12, Title 22 of the California Code of Regulations the following deficiencies are cited on see LIC 809-D. Appeal Rights provided during inspection. Exit interview was conducted with Director, Sandra Dermer.
