25 | An unannounced case management visit was conducted today by Licensing Program Analysts (LPA)s Caroline Harris and Candis Rodriguez. Met with April Gooboian. A census was taken. The purpose of todays visit was to clarify the use of the breezeways, across from the 3 year old and 4 year old classrooms.
On 1/29/19, LPA Michael Duarte conducted a case management inspection in order to increase the facilities capacity. The breezeways, across from the 3 year old and 4 year old classrooms, where there are long tables attached to the walls, were included as part of the increase and can be used for classroom activities, including eating times, snacks, lunch. The licensee stated that it was a multi purpose area and they used it for activities and sometimes an eating area. LPA Harris confirmed that the area is part of the license and can be used for the children. Licensee made changes to the facility sketch.
Upon arrival, the LPA's observed an operating bulldozer in the front of the facilities office where people passed by. While conducting the walk through, the LPA's observed two sections of the rod iron fencing behind the 3's classrooms to be down causing an opening to a dirt field. The mesh fencing between the playground and the area where they were bringing in dirt with a bulldozer was also down. The LPA's also observed children and teachers exiting out the back of the 3's classrooms and children were in the playground.
Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, the following deficiencies are found: (see LIC809-D) Licensee was provided a LIC 9213 Notice of Site Visit form that is required to be posted for 30 days. A copy of this report was also provided to the licensee along with appeal rights, and shall be made available to the public upon request. Exit interview was conducted with April Gooboian. |
