On 4/14/2022, Licensing Program Analyst (LPA) provided complaint findings regarding the above allegations to Director Michele Van Ornum.

During the course of the investigation, LPA conducted interviews with parents and staff. The investigation revealed that on 8/13/2021 mold was found in the infant classroom. Staff failed to notify Community Care Licensing (CCL). Additonalyy, authorized representatives were not notfied in accordance with Title 22 regulation 101212(f).

During the COVID-19 pandemic, parents remained outside during pick up and drop off of their children. Staff were signing children in/out from the center which is a requirement from only the authorized representative.

Based on the information obtained during the investigation, staff did not notify parents of mold in the facility and staff do not maintain accurate records. The preponderance of evidence standard has been met, therefore, the above allegations are found to be substantiated.

Per California Code of Regulations Title 22 Division 12 Chapter 1, the following deficiencies are being cited on this report (See LIC9099-D).

An exit interview was conducted and report was reviewed with the Director MIchele Van Ornum. A copy of Appeal Rights and the Notice of Site Visit was given and must remain posted for 30 days.

Director agreed to develop a written plan addressing how she will ensure parents/authorized representative and not staff sign in/out day care children.
