Licensing Program Analysts (LPAs) Theresa Marquez and Stephanie Navarro conducted an unannounced Annual/Random inspection and met with Lead Teacher and Erica Murillo and substitue teacher Elizabeth Gomez. LPAs toured the facility, both indoors and outdoors. There are no bodies of water on site. Firearms/weapons are not allowed or stored on premises. There is a ratio of one teacher supervising no more than 12 children in attendance. During inspection LPA Marquez observed Child #1 exiting the classroom restroom alone with the restroom door closed. Teacher #1 was southeast of the restroom and Teacher #2 was south of the restroom when Child #1 was observed. This resulted in Child #1 having no supervision. Disinfectants, cleaning solutions and other dangerous items shall be inaccessible to children. No poisons were observed during today’s inspection. All materials and surfaces accessible to children are toxic free. Medications are stored in locked cabinets in the school office and inaccessible to children. All toilets, hand washing, and bathing facilities are in safe and sanitary operating conditions. All floors are clean and safe. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Meals are provided and prepared by the school cafeteria. Solid waste storage vessels, including moveable bins, have tight-fitting covers on, are in good repair. Uncontaminated drinking water is available both indoors and outdoors. Menus are posted at least one week in advance, where an authorized representative can view them. Facility has a functioning carbon monoxide detectors that meets statutory requirements. Playground equipment is in good condition, free of sharp, loose, or pointed parts.
Outdoor activity space surface is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings, and slides have cushioning material to absorb falls.
Before working in a licensed child care facility, all individuals subject to a criminal record review have a clearance or exemption and is a condition of employment with Fresno Unified School District. During inspection, the front office,Teacher and Program Director were unable to provide a personnel file for Substitute teacher Elizabeth Gomez . The personnel file for Teacher Erica Murillo contained all appropriate documentation of education credits.

At least one person trained in CPR and Pediatric first-aid is present when children are at the facility or at off-site activities. Staff was unable to provide evidence of AB1207 Mandated Reporting training requirement. The person, who signs the child in/out, is responsible for the child, uses their full legal signature and records the time of day. Child's admission agreement is available for review. Staff records contain appropriate, documentation of education credits. This is an AM/PM half day program which operates on a traditional school year schedule. The morning session is 8:00am to 11:00am and the afternoon session is
11:30pm to 2:30pm, Monday through Friday. Required CCL forms are posted on parent's board.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, the following deficiencies are found: (see LIC809-D) Site Visit Notice posted on the parent board.
