Licensing Program Analyst (LPA) Brannon met with director, Ermelinda Perez. Previous visit conducted on 11/1/19 was due to change in capacity. Licensee requested extensions on due date. LPA granted the extension. LPA conducted today's visit to verify the corrections were made. The following corrections are to be corrected by March 5 2020. LPA will conduct a return inspection visit.
School Age Classroom
1) Chipping paint on the interior walls
2) Baseboards need to be replaced in an area.
3) Blinds need to be replaced.
School outside play yard
4) Cement ramp leading from preschool yard to school yard is ragged.
5) Black top area has deep cracks and are a tripping hazard.
6) Fencing on the black top area is missing a pole and fencing needs to be repaired correctly or replaced.
7) Gate leading to empty pool needs to be replaced/repaired correctly. Upon return visit, LPA will open the gate to ensure gate is self-latching. Locking gate does not meet Title 22 requirements.
8) Blacktop is missing a large piece. This is a tripping hazard.
9) Tree roots need to be covered. They are a tripping hazard. There are tree roots exposed in the grass area.
10) Bushes along fencing needs to be trimmed.

11) Fencing along Sierra Madre needs to be repaired correctly or replaced.
12) The wooden box was removed from the outside play yard. Now there is a pipe sticking up. This is a potential hazard to children and must be made inaccessible to children. Licensee shall come up with a plan to make this inaccessible. An orange cone will not be sufficient.
13) The pool gate fencing is damaged and needs repair.
Upon correction of the above items and pending final review, a recommendation to license facility for 21 school age children will be made. Exit interview with director, Ermelinda Perez was conducted.
