On 12/21/2023, Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted a Case Management. The purpose of the inspection was to review the Plan of Correction for the deficiencies cited on 11/9/2023. Today, LPA met with Director, Sandra Graham. LPA toured the facility, and a census was taken of 8 preschool children and 2 school age children.

During the initial tour LPA observed that Staff 3 and Staff 5 were in the preschool classroom with a census of 10 children. LPA observed that 2 out of the 10 children were school age children. LPA discussed with Director, Sandra Graham that preschool children and school age children can not be commingling, and that school age children need to be in the school age classroom with a qualified staff who will be supervising and caring for children. Director, Sandra Graham stated that she understood. LPA observed as Director moved school age children from preschool classroom and asked children to sit and play with toys in the common dining area and them moved children to school age classroom.

Exit interview conducted and report was reviewed with Director, Sandra Graham. Appeal rights were provided.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).

This report shall be made available to the public upon request.

LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Sandra Graham stated that facility will write a letter stating how the center will ensure that school-age children and preschool children are not commingling. Sandra stated that letter will be submitted to the Department by the POC due date 12/29/2023.

This requirement was not met met as evidence by: LPA observed 2 school-age children inside the preschool classroom. LPA reviewed with Director, Sandra Graham that children can not be commingled, which poses/posed a potential health, safety or personal rights risk to persons in care.
