On 9/5/2024, an unannounced complaint inspection was conducted by Licensing Program Analyst (LPA), Claribel Soto. LPA met with Site Supervisor, Irene Carrillo. LPA toured the facility and census was taken. The purpose of today's inspection is to close the complaint investigation. During the course of the investigation, LPA interviewed staff, parents and reviewed and obtained facility records.

The allegation of due to lack of supervision, child received an injury was investigated and based off interviews and record review there is no indication that a child received an injury due to lack of supervision. Although the allegation may have happened or may be valid, there is not a preponderance of the evidence to prove the allegation occurred.

The findings with regard to the above allegation are UNSUBSTANTIATED.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency is cited.

An exit interview conducted with Site Supervisor, Irene Carrillo. A Notice of Site Visit was posted on parent board. This report shall be made available to the public upon request. Appeal Rights were provided.
