On 08/02/2021, Licensing Program Analysts (LPAs) Luisa Gavoutian and Cynthia Brannon conducted an unannounced Annual Inspection for the infant license. LPAs met with Licensee Nancy Fuller, and toured the facility indoors and outdoors. Hours of operation are Monday – Friday, 6:30 a.m. – 5:30 p.m.

All children are under supervision, including visual supervision, of a teacher at all times. There is a ratio of one teacher supervising no more than four infants in care. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPAs observed two holes in the plaster of the wall separating the infant play area with the crib room. The holes were low enough and big enough that infants can stick fingers or hands in. Director of Operations Rebecca Lembert stated that the facility has plans to replace the whole wall in the future but will fill the holes in with stucco temporarily until the wall is replaced. The facility is free of flies, insects and rodents. There is a working carbon monoxide detector. The facility has sufficient age appropriate furniture, and equipment including cribs, cots or mats, changing tables and feeding chairs. The facility has indoor activity space for infants that is physically separate from space used by preschool child care center/school-age child care center components.

Each crib, mat, or cot is occupied by only one infant at a time and cribs are kept free from all loose articles, and there are no objects hanging above or attached to the crib. Upon entering the crib area, LPA Gavoutian observed one infant asleep in a crib who had a blanket underneath. (Continued on LIC 809-C)

LPA Gavoutian discussed with Lead Teacher Kristina Harmon the requirement to remove all loose articles, including blankets, from infant cribs. Lead Teacher Harmon stated that the infant had just fallen asleep in her arms and she had placed the infant down into the crib with the blanket. Lead Teacher Harmon immediately removed the blanket from underneath the infant. Infants are not swaddled while in care. Staff physically check on sleeping infants every 15 minutes and document any signs of distress, including, but not limited to, flushed skin color, increase in body temperature, restlessness and labored breathing. Documentation for infants up to 12 months includes sleeping position if it is other than on their back. Individual Infant Sleeping Plan is completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping. Staff-infant ratio requirements are being met while infants are sleeping. During today’s inspection, LPA Gavoutian observed three staff supervising six infants in care.

Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA Gavoutian observed that a section of the infant play area is temporarily being used as a storage area. LPA observed some mats, napping cots, rolling storage shelves, book shelves, and picnic tables in this area. Licensee stated the items will be moved into a storage unit or sold in the future. Licensee stated, and LPA observed, a note for staff reminding staff that children are not to have access on that side of the play yard. Additionally, Licensee stated that due to the extreme heat of 103-105 degrees F, the infants have not been going outside to play very often. LPA Gavoutian observed proper supervision being provided and a ratio of one staff to two infants being maintained. No high climbing equipment was observed.

All kitchen, food preparation, and storage areas are clean, free of litter/rubbish and rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Uncontaminated drinking water is available both indoors and outdoors. Menus are posted at least one week in advance where an authorized representative can view them.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to criminal record review have obtained a clearance or criminal record exemption. Community Care Licensing (CCL) shall notify a Licensee to immediately terminate the employment of, or to remove/bar any person with specified convictions or for other reasons; the Licensee shall comply with the notice.

Capacity and limitations as specified on the license are being maintained. All children are under supervision, including visual supervision, of a teacher at all times. There is a ratio of one teacher supervising no more than four infants in care. At least one person trained in Pediatric CPR and First Aid is present when children are at the facility or at offsite activities. The name of the childcare center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. This facility maintains an electronic and paper sign-in/out system. On this date, LPA Gavoutian observed one child had not been signed in by their authorized representative today. Licensee stated the authorized representative who dropped the child off this morning did not have a face covering and was not allowed into the facility to sign the child in.

LPAs reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and/or others who can assume responsibility if the authorized representative cannot be reached. All children’s files were complete with a medical assessment and individual feeding plan. LPAs reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis, and measles, and documentation of completed mandated reporter training. Staff records contain documentation of meeting qualification requirements.

This facility provides Incidental Medical Services – IMS. No children requiring IMS are currently enrolled. LPAs and Licensee discussed the CCL website (www.ccld.ca.gov) which provides access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, the following deficiency is found: (see LIC809-D). Exit interview was conducted with Licensee Nancy Fuller. Licensee was provided a copy of appeal rights.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is required to be posted for 30 days.
