25 | On 02/04/2025, Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced Case Management - Incident inspection. LPA toured the facility, took a census, and met with Director of School Readiness Christina Coble to discuss an incident from 01/16/2025.
An Unusual Incident Report (UIR) was submitted to the Fresno Community Care Licensing Office regarding child #1, who fell from a playground structure and broke their right arm. Staff reported that on the day of the incident, at approximately 4:30 PM, there were 8 children outside with two staff members. Child #1 fell approximately 3.5 feet from a playground rock climbing playground structure. Staff were supervising while managing afternoon pickups.
After the fall, staff escorted child #1 to the classroom, provided first aid, and applied an ice pack as child #1 was crying. The child displayed discomfort, and upon pickup, the mother took them for medical attention, where a fracture below the elbow was confirmed.
LPA observed the playground and verified adequate wood chip and padding coverage for fall mitigation. The child’s mother requested that they avoid the slide until healed. Child #1 returned to care on 01/21/2025 with a cast and sling. The incident was determined to be isolated and not due to a lack of supervision.
Per Chapter 1, Division 12, Title 22 of California Regulations, no deficiencies were cited. An exit interview was conducted with Director Christina Coble. A Notice of Site Visit was provided and must remain posted for 30 days. Appeal Rights were provided.
