A prelicensing inspection visit was conducted on this date by Licensing Program Analyst (LPA) Brannon and Licensing Program Manager (LPM) Juarez, who met with licensees Kylie Barnett and Keri Frazier. The center is located at North Fresno Church, in the back. The licensee is requesting the following capacity: 58 Preschool children. This program will operate year-round, daily from 7:30 AM to 5:30 PM. Breakfast, lunch and snacks will not be provided. Parents/legal representative will be providing meals and snacks. Parents will provide drinking cups for children to provide indoor drinking water. The children’s cups will be labeled. Every day, children will bring their cups, staff will label the children’s cups and the children will take the drinking cups home. Licensee will provide drinking cups and food to children when water and/or food was not provided by parents/legal representatives. The outside water is provided by a pitcher with a disposable cup dispenser and disposable cups. Ill children and staff will utilize the bathroom located in the lobby on the first floor. Ill children will be isolated in nursery sleeping room, located on the first-floor lobby, until parent/legal representative picks up ill child. Room measurements taken and reviewed with licensee, Keri Frazier. There are five classrooms and one activity room that will be used by preschool children. The total preschool square footage is 2863 which will accommodate the requested capacity of 58 preschool children.

Adequate storage space available for children's belongings. There is no outdoor storage available for toys and equipment. Toys and equipment are age appropriate. During today’s inspection, licensee removed an infant changing table from lobby to the staff bathroom, located in the lobby. LPAs observed two carbon monoxide detectors; one downstairs and one upstairs. The elevators are locked, secured, inaccessible to children and adults who are not fingerprint cleared and/or associated to facility. The stairwell used by the public are locked from the outside and have alarms installed. The stairwell used by the public is accessible from the outside, without entering the licensed preschool area.

Outdoor measurements taken on this date total 4393 square feet which will accommodate the requested capacity of 58 preschool children. Adequate shade is available in the outdoor activity area. Licensee is utilizing a shade structure over the climbing equipment and trees as they mature. The applicant is using rubber blocks for cushioning under the outdoor climbing structure. LPA informed licensee that the rubber blocks may constrict, leaving open areas between the rubber blocks. This can become a tripping hazard. Per licensee, one staff will be placed next to the climbing structure to provide adequate visual supervision. The climbing structure does have a tube component. Licensee will ensure that staff will be positioned to provide visual observation of children in the tube component. The trees have black plastic surrounding the tree and covering the tree roots. Per licensee, supervision provided by staff will ensure children do not trip over the plastic around the trees. There is a permanent pipe that is laying on the cement. To ensure the permanent pipe does not become a tripping hazard, licensee shall ensure the red cone is stationed before children can use the outside play area.

Currently, the children’s restrooms are located on the first floor. Staff will walk children from the second floor to the first floor, to use the children’s restroom, as needed. Staff are required will be in the children’s bathrooms when in use by day care children. Currently, licensee is utilizing plastic step stools to be able to reach the sinks. LPA is recommending that the step stools be replaced by broader step stools. There are 4 toilets, 2 urinals and 4 sinks/hand washing fixtures in the children's bathrooms which will accommodate the requested capacity of 58 preschool children. Children will be utilizing drinking cups, provided by parent/legal representative. A water pitcher with disposable cup dispenser and disposable cups will be utilized for outdoor drinking water.

The fire clearance has been received and approved for the capacity of 58 preschool children. The fire clearance was received on May 20, 2021.

Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. There are windows on the first floor that are kept locked. Licensee is aware that the windows can be opened when screens are installed. The windows on the second floor are not operational. The children’s cubbies are located outside of the classrooms. The classrooms are not to be used as storage. There are non-operational windows located on the second floor.

Pending a final file review, a recommendation will be made to license the above facility for a capacity of 58 preschool children. Please update LIC 500, LIC 610, LIC 308, Facility Sketch upon licensure.
