25 | Prior to today's inspection, LPA telephoned call applicant and conducted the COVID-19 Emergency Response Tele-Inspection Screening Process. On May 24, 2021, Licensing Program Analyst (LPA), Norma Lomeli met with Applicant, Linda Whitbey for a pre-licensing/ change of location inspection. Applicant and her husband reside in the home. Licensee’s Assistant will be Denise Thigpen who completed the CPR/First Aid training through Pediatric Plus with Emergency Medical Services Authority stickers (EMSA) and expires on May 8, 2023. Background clearances are discussed and LIS 531 is signed indicating that the adults currently living in the home and/or providing care and supervision to children have a criminal record clearance. Fire clearance was granted on May 19, 2021.
Facility was inspected inside and outside as shown on the facility sketch and the following items were discussed:
- Fire clearance was received on May 21, 2021. Fire alarm is located in the downstairs hallway wall that is located near the downstairs bathroom.
- This is a two story, four bedrooms and two and a half bathrooms home and children will have access to the living room, dining room, kitchen, family room and downstairs bathroom. There is a gate at the bottom of the stairs making upstairs area inaccessible. Off-limits rooms are made inaccessible by use of plastic door knob covers and a child proof safety gate.
- There is a fireplace in the family room that applicant states it will not be used during day-care hours.
- LPA observed children size furniture, safe toys and books for the children. Children will nap in the living room on mats. Applicant states that she does not provide care for infants. Applicant understands she is to supervise children at all times.
