On 11/01/2019 from 9:00am thru 12:15pm Licensing Program Analyst (LPA) David Wilson conducted a Case Management unannounced inspection due to this agencies timely self reported Unusual Incident Report (UIR). The incident date was 10/31/2019 and was in regards to a report of an alleged inappropriate staff and child interaction involving possible personal rights violations. On 11/01/2019 LPA toured the facility, interviewed four staff and reviewed and obtained applicable records. Due to children's ages in this program none were interviewed. During the 11/01/2019 inspection, LPA observed that adequate care and supervision was being provided including that staffing ratios were being met and the facility was operating within the licensed capacity. On 11/01/2019 LPA determined this Case Management must be continued pending further inquires into the above reported UIR. On 11/04/2019 while not at this facility, LPA conducted a confidential interview of a witness. On 11/15/2019 while not at this facility, LPA conducted a confidential interview of another witness.

On this date of 11/15/2019 from 1:00pm to 1:45pm LPA arrived unannounced for a follow up to this Case Management. LPA toured the facility. During this 11/15/2019 Case Management inspection, LPA observed that adequate care and supervision was being provided including that staffing ratios were being met and the facility was operating within the licensed capacity.

Regards to the above mentioned alleged inappropriate staff and child interaction, LPA's Case Management concludes that there is no corroboration, nor evidence otherwise including none of LPA's observations and document reviews. With this Case Management's evidence, it could not be determined of any violations of a child's personal rights.

This report was reviewed and discussed with Site Supervisor Barajas-Cruz. All licensing reports are public information and must be made available upon request for at least three years. Notice of Site Visit shall be posted for 30 days from today's visit. There were no Title 22 deficiencies cited during today's inspection
