25 | On 11/13/2019 at 10:45am Licensing Program Analyst (LPA) David Wilson arrived at this facility to continue a Case Management (CM) inspection due to this agencies timely self reported written Unusual Incident Report (UIR). The incident date of UIR was 10/25/2019. This UIR was in regards to an alleged inappropriate child to child interaction. LPA's first inspection of this CM UIR inspection was on 11/01/2019 (see LIC809 11/01/2019). The purpose of the CM is to determine if adequate care and supervision is occurring at this facility.
During the 11/01/2019 unnanounced inspection, LPA toured the facility, interviewed staff and reviewed and obtained applicable records. On 11/01/2019, there were no children in attendance due to a staff administrative day.
On 11/05/2019 for this CM, LPA conducted a telephone interview with a confidential witness.
During this 11/13/2019 unannounced inspection, LPA toured the facility, interviewed four children and observed the classroom and outdoor play area. During this date's inspection, staffing ratios were being met and there were eighteen children being supervised by five teachers/aides. The facility was operating within the licensed capacity.
LPA's observations, interviews and documentary evidence reviews conducted at this facility during both of the above mentioned inspections found no evidence or corroboration of any lack of constant visual supervision. All staff and child interviews found that children at this facility are being safely supervised and adequately cared for.
This report was reviewed and discussed with Director Aisenbrey. All licensing reports are public information and must be made available upon request for at least three years.Notice of Site Visit shall be posted for 30 days from today's visit.
