Licensing Program Analysts (LPA) Kiriko Lynch and Noah Wheeler conducted a Plan of Correction (POC) visit in response to multiple violations cited during a visit at the facility on 04/26/2024. During the visit conducted today on 05/09/2024, LPAs met with Owner/Licensee Rose McCutchen to discuss plan of corrections, and extend the due dates of previously cited deficiencies to be due by 06/01/2024. Owner/Licensee agreed to submit POCs in writing to the Department by 06/01/2024. Exit interview conducted with Rose McCutchen, notice of site visit was posted, and appeal rights were provided.

Notice of Site Visit must be posted for 30 days.
