Licensing Program Analyst (LPA) Yolanda Baez arrived at the facility to conduct an annual random inspection. Upon arrival LPA Baez met with Teacher, Laura Fimbres. The following ratios were observed:

Primary 1 (Is a Preschool classroom, Serves "Toddlers" 2 to 3 years of age):
  • There were not any children or staff members present throughout the inspection
Primary 2 (Is a Preschool classroom, Serves children 3 to 4 years of age):
  • There were 9 children present with 2 staff members
Pre-Kinder (Is a Preschool classroom, Serves children 4 to 5 years of age):
  • There were not any children or staff members present throughout the inspection
Kinder Class (Is a Preschool class, Serves children 5 to 6 years of age):
  • There were not any children or staff members present throughout the inspection
Appropriate ratios and capacity were observed.

Furniture and age appropriate equipment is in good condition indoors. Children's toilets and hand washing facilities are sanitary. Rooms are safe and clean. The parents are responsible for providing lunch and the center provides snacks. Staff at the facility are responsible for assisting children with the lunches that are provided from home. Drinking water is readily accessible inside of the classroom through the use of children's water bottles that are brought from home. Children have access to water outside of the classroom through water jugs and disposable cups. All disinfectants, cleaning solutions, and other hazardous items are inaccessible to children through latches and locks. Storage area for poisons is locked. Solid waste storage vessels, including moveable bins, have tight-fitting covers on, and are in good repair. Outdoor play area is fenced with sufficient material for cushioning, sufficient amount of toys, area has canopies used for shade, and it is surrounded by a 4 foot high fence. There are several toy structures and playhouses located outside on the play yard that are broken and have sharp points sticking out, see 809D for cited deficiency. There is an operational smoke detector and carbon monoxide detector at the facility. The last emergency drill was conducted on 03/2019. Some children's files were reviewed and are in compliance. Staff files were not able to be located because Ms. Fimbres stated that she is not sure where the files are located. Therefore, staff qualifications and transcripts could not be reviewed today. LPA reviewed the following with Ms. Fimbres: IMS, SIDS, Car seat Law, Shaken Baby Syndrome, and the new Safe Sleep Regulation. This facility does provide Incidental Medical Services- IMS and understands that a written Plan of Operation must be submitted as soon as . Immunization law (SB792) was discussed with Ms. Fimbres and she understands that anyone who provides care and supervision to the children must have immunization records maintained at the facility for: pertussis, measles, and influenza. Ms. Fimbres was reminded that all staff are to have some type of immunization record/ proof of immunity present at the facility for review, staff files were not able to be located and SB792 could not be reviewed.

LPA Baez discussed the new Mandated Reporter training, AB1207. LPA Baez reminded Ms. Fimbres that all staff members are to take the training and have the printed certificates present at the facility and available for review. The mandated reporter training per AB1207 may be located at www.mandatedreporterca.com. Staff files were not able to be located and AB1207 could not be reviewed.

NOTICE OF SITE VISIT IS TO BE POSTED FOR 30 DAYS. LPA observed the posting the Notice of Site Visit.

Ms. Fimbres stated that she will show the report to Director, Claudia Gallegos, and will discuss a plan of correction. Plan of correction will be communicated to LPA upon Director's return from vacation, after 07/31/19. deficiency must be corrected by due date of 08/13/19 and POC visit will be conducted after 08/13/19.
