On December 5, 2023 at 12:10 PM, Licensing Program Analyst (LPA), Gloria Gonzalez conducted an unannounced Plan of Correction (POC) inspection. The purpose of the inspection is to ensure the citations issued on 11/28/23 were corrected. Upon arrival, LPA met with Staff Member, Mia Molina and was led on a tour of the facility. Director, Claudia Gallego arrived on or about 12:20 PM. There were seventeen (17) children and three (3) staff members present.

LPA observed facility operating within ratio.  Classroom P1 had a ratio of nine (9) daycare children and two (2) staff members and classroom P2 had a ratio of eight (8) daycare children and one (1) staff member. LPA reviewed children's records to be complete with the LIC9224 acknowledgement forms. LPA also reviewed the most current children's sign in/out sheets and staff time sheets and found facility to be within ratio. Director provided LPA with a current LIC500. The citation issued on 11/28/23 for facility being out of ratio has been corrected.

Director had provided a picture of the playground slide structure in the trash bin on 11/30/23. LPA observed the play structure with a slide was removed from the playground and no longer on site at the time of this inspection. LPA also observed the 3 panels in classroom P2 have been replaced. Director will submit in writing stating how the facility will ensure the playground and classrooms will be maintained in safe conditions. The citation issued on 11/28/23 for the facility not maintaining equipment in a safe condition has been corrected.
 
A copy of the report and appeal rights (LIC 9058) was provided to Director. LPA observed Licensee post LIC9213 – Notice of Site Visit and Licensee was advised this notice is to be posted for 30 days from today’s date.  Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted and report was reviewed with the licensee, Claudia Gallego.
