On 6/5/2023 at 10:30am, Licensing Program Analyst (LPA), Vicky Williamson conducted an unannounced annual required inspection. LPA met with Site Supervisor, Elisa Mendez and disclosed the purpose of the inspection. LPA toured the inside and outside of the facility. This program operates from August to June each year. Days and hours of operation are Monday - Friday, morning session is from 8:00am - 11:00am and afternoon session is from 12:30pm - 3:30pm.

Furniture and equipment are in good condition. Playground equipment is in safe condition. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. High-climbing equipment has sufficient cushioning. Toilet and hand-washing equipment are in safe and sanitary operating condition. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair. Disinfectants, cleaning solutions and other hazardous items are made inaccessible to children. Site Supervisor stated there are no bodies of water on the premises. LPA did not observe any bodies of water. Site Supervisor stated there are no firearms or ammunition allowed or stored on the premises. Drinking water is available both indoors and outdoors. Facility has functioning carbon and smoke detectors that meet statutory requirements. The last fire/disaster drill was conducted and documented on 5/4/2023. The facility provides breakfast and lunch that is prepared by the school cafeteria. Menus are posted monthly.
A review of staff records on this date indicates facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Site Supervisor was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility of off - site activities. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening and immunization records. Mandated Reporter training certification for Staff 1 (S1) expired on 2/15/2022. LPA informed Site Supervisor to ensure the mandated reporter training is completed once every two years.

This facility provides Incidental Medical Services – IMS, however Site Supervisor stated that no children require medications at this time. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA and Site Supervisor discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, mandated reporter training, and California Megan’s Law (www.meganslaw.ca.gov).

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiencies are being cited: See LIC 809D. An exit interview was conducted with Site Supervisor, Elisa Mendez, and a copy of this report, Appeal Rights and Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Notice of Site Visit posted on the bulletin board in the classroom.
