25 | Licensing Program Analyst (LPA) Yolanda Baez arrived at the facility to conduct a case management inspection. Upon arrival LPA met with Director, Isabel Garcia. Ignacia Avila arrived within 25 minutes. There were 48 preschool children present during today's inspection, appropriate ratios and capacity were observed.
The purpose of today's inspection is to issue a Type B deficiency for the incident that was self reported on 10/23/2019 and first investigated on 11/20/2019. Based on further interviews that were conducted and the center's own admittance, it was determined that a child was left unattended from 3 to 5 minutes in the child care bathroom that is located inside of the church that is on the same property. The child that was left unattended was found by a church member and returned back to her classroom that was on the playground at the time, please see 809D for cited deficiency. In addition, LPA reviewed staff files from the annual inspection that was conducted on 11/20/2019 to clear the following deficiency sections that were issued:
- 101217(a) Personnel Records. Personnel records shall be maintained on the licensee, administrator, and each employee, and shall contain specified information. (13) A signed statement regarding their criminal record history as required by Section 101170(d).
- HSC 1596.7995(a)(1): Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles...
Title 22 regulation section 101217(a)(13) and Health and Safety Code section 1596.7995(a)(1) have both been cleared effective today.
