25 | On February 19, 2020 at 11:00am a Case Management inspection conducted by Licensing Program Analyst Michelle Palacio and Licensing Program Manager Tulam Vu and met with Center Director Christina Tanori. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Center Director. There were 12 children in the main area being supervised by two staff members. Nine children were observed in the room 4/5 with two staff. A review of staff records on 02/19/2020 indicates that all facility staff or other individuals who require caregiver background checks have received criminal background and child abuse index clearances or exemptions.
Center Director submitted a written request to the Department to inspect the temporary playground due to the old playground being demolished. Temporary playground is located to the west of the church entry. The playground was inspected and measured today at 2356 square feet which can accommodate 31 children. The play yard is fully fenced with canopy shade and drinking fountain. Director stated additional cushioning will be added around and under the play structures, swing tires, and wooden beams will be sanded and painted to prevent splinters. Director stated at this time the temporary playground is not in use. Director will submit pictures with corrections to the Department prior to children utilizing the playground.
Director stated that the permanent playground will be completed by next school year. Licensing will be notified once the playground has been completed and provided with an updated facility sketch. An inspection is required prior to use.
No deficiencies cited. An exit interview was conducted with the licensee. The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. LPA provided notice of site visit and observed it being posted at the facility. |
