 | On November 10, 22 at 10:30 AM, Licensing Program Analyst (LPA) Gloria Gonzalez conducted an unannounced Case Management inspection to follow-up on a self-reported incident that occurred on or about during the week of 10/17/22-10/21/22 which was first reported to the facility on 11/1/22.  LPA disclosed the purpose of the inspection and was granted a tour into the facility by Site Supervisor, Joanna Navarrette. Site Supervisor, Isabel Garcia arrived on or about 11:00 am and continued the visit with LPA. There were 24 children and 11 staff members at the time of the inspection. 
 Facility called the Licensing duty line on 11/3/22. An unusual incident report (UIR) was submitted to the San Diego Regional Office (SDRO) on 11/7/22 alleging a possible personal rights violation. The incident occurred on or about the week of 10/17/22-10/21/22. LPA conducted interviews with the Site Supervisor, staff members, and several daycare children. Based on the information obtained the incident requires further review.
 
 Ms. Garcia was provided a copy of the appeal rights (LIC 9058 12/15).  LPA provided the notice of site visit (LIC 9213) and observed it being posted at the facility.
 
 An exit interview was conducted with Site Supervisor, Isabel Garcia.
