On July 20, 2021, at 01:20 p.m., Licensing Program Analyst (LPA) Gloria Gonzalez and Licensing Program Manager (LPM) Tulam Vu, conducted an unannounced Annual Inspection and met with Interim Program Director, Isabel Garcia. LPA disclosed the purpose of the inspection and toured the facility indoors and outdoors. The program currently operates in three classrooms A, C, and E. Classroom B is currently not in use due to low enrollment. Classroom E operates as part time 8:00am-11:30am and 12:30pm to 4:00pm. Classroom A and E operate as part time AM session from 8:00am to 11:30am and PM session from 12:30pm to 4:00pm serving ages 3-5 yrs old. The following ratios were observed:
Classroom E had 5 Children with 2 fully qualified teachers.
Classroom A had 5 Children with 1 fully qualified teacher and 1 Aide.
Classroom C had 11 napping children with 1 fully qualified teacher.
Title 5 capacity and limitations as specified on the license are being maintained.

Director stated there is no swimming pool or other bodies of water on the premises. Director stated there are no firearms or ammunition allowed on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible.

Furniture and equipment are in good condition. Facility has two playgrounds available for children's use. Playground equipment is in safe condition. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. During the inspection the playgrounds are not being used due to weather condition. Toilets and handwashing facilities are in safe and sanitary operating condition. Two of the toilets in Classroom A are being repaired. Facility has sufficient toilets and sinks available for childrens use. Floors in the facility are clean and safe. Kitchen, food preparation and storage areas are clean. Food was inspected and was observed against contamination. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have rubber matting cushioning material to absorb falls. Carbon monoxide detectors in each classroom meet statutory requirements.

A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Upon notification from the Department, the Director will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Opening and closing staff members have current CPR and Pediatric First Aid valid through June 2023. LPA informed Isabel Garcia that the name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence shall be reported to the Department within 30 days. Sign in and sign out sheets were reviewed showing the person who signs the child in/out of the facility are using their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a Title five ratio of one teacher supervising no more than 8 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Menus are posted at least once a month in advance where an authorized representative can view them.

At this time the facility does not provide Incidental Medical Services (IMS).

Director is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, California Megan’s Law www.meganslaw.ca.gov), Lead Poisoning Facts, Forms and Regulations.

No deficiencies are cited. An exit interview was conducted with Ms Garcia. Ms Garcia was provided a copy of their appeal rights (LIC 9058)

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
