Licensing Program Analysts (LPAs) Yolanda Baez and Tresha Souza arrived at the facility to conduct a pre-licensing inspection. LPAs met with Applicant Wendy Marquez and Program Manager/ Administrative Liason, Ashley La soya. This Preschool program operates in the classrooms 1, 2, 3, 4, 5, 6, 7, 8, 9, and 10. The hours of operation are Monday - Thursday from 7:00am to 5:30pm and from 7:00am - 3:00pm on Fridays. This center is located within a shopping center on the intersections of N. Imperial Ave and Bradshaw Ave.

Classrooms 1, 2, 3, 4, 5, 6, 7, 8, 9, and 10 were inspected and measured during this inspection and measure the following:
  • Room 1: Preschool Class (ages 2 to 5 years): measured 604.29 square feet
  • Room 2: Preschool Class (ages 2 to 5 years): measured 606.71 square feet
  • Room 3: Preschool Class (ages 2 to 5 years): measured 602.57 square feet
  • Room 4: Preschool Class (ages 2 to 5 years): measured 606.71 square feet
  • Room 5: Preschool Class (ages 2 to 5 years): measured 602.57 square feet
  • Room 6: Preschool Class (ages 2 to 5 years): measured 606.71 square feet
  • Room 7: Preschool Class (ages 2 to 5 years): measured 606.71 square feet
  • Room 8: Preschool Class (ages 2 to 5 years): measured 602.57 square feet
  • Room 9: Preschool Class (ages 2 to 5 years): measured 599.16 square feet
  • Room 10: Preschool Class (ages 2 to 5 years): measured 596.75 square feet

The total measurements for all classrooms measured 6,034.75 square feet which is sufficient to accommodate 172 children. All classrooms are equipped with appropriate furniture and napping equipment (cots) including tables, chairs, cubbies, bookshelves, and other materials for the children. There are a total of 27 toilets, 4 urinals (one is not currently working), 1 changing table, and 31 sinks available for the children's use, which is sufficient to accommodate the requested capacity of 160 children. ...Continuation on page 2...

Applicant provided the following pending documents at the time of the inspection: Sample menu (need to add portions), please add a second temporary relocation site to the LIC 610 (must be about 2 miles away AND please include a phone number), On page 4 under "Our Program" section please make the changes to the ages that you serve. Please also make the discussed changes to your Incidental Medical Services (IMS) Plan of Operation. Applicant provided the Board minutes to designate her as the Applicant for the organization. In addition the following was received for Wendy Marquez:
  • LIC 503 that verifies health and TB test requirement
  • copy of immunization records to show immunity against Pertussis, Measles, and Influenza to show compliance for SB792. If you decide to opt out of the Influenza vaccine, please provide a declaration that is signed and dated
  • proof of completion of the mandated reporter training to show compliance with AB1207.
  • LIC 501: Personnel Record
  • CPR/FA certificate
  • Component I (Application) & Component III (Record Keeping) orientation certificates
  • Copy of transcripts OR Site Supervisor Permit

The following corrections are needed for the Incidental Medical Services (IMS) Plan of Operation:
  • Add records that the center will maintain for Children and Staff
  • Plan for needed safety precautions
  • Transporting of items needed to care for this condition for activities/field trips away from the facility
  • A plan for updating the physician instructions annually

Drinking water is available in all of the classrooms via water jugs and disposable cups in each classroom. There are 3 separate bathrooms for staff across the hallway from the children's restrooms. The women's staff bathroom consists of 5 toilets and 2 sinks and the men's staff bathroom consists of 3 toilets, 2 urinals, and 2 sinks. There is a staff coed restroom that consists of 1 sink and 1 toilet.

There is a nurse's office on site that consists of one toilet, one changing table, and one sink and will be used as an isolation bathroom and area for children who are ill. Disinfectants, cleaning solutions, and other hazardous substances are kept in locked cabinets. The kitchen area currently includes 3 sinks, 1 large refrigerator, 1 large freezer, storage units, 1 grease trap (required by the city), 4 carts (to transport food to and from the classrooms). Applicant stated that 2 food warmers are to be delivered by 07/19/19. The menu is posted. The center will be providing hand written sign in and sign out. All required documents are posted. There is a carbon monoxide detector, smoke alarm, and first aid kit with required supplies.

The facility has a Preschool playground that is located directly outside of the "Rear Lobby" (as stated on the facility sketch) and was measured at 6,241.02 square feet, which is sufficient for only 83 Preschool age children at one time. The Preschool playground is complete with age-appropriate play equipment, shade area, sufficient material for cushioning, and is surrounded by a 4 foot high fence. Children have access to water outdoors via water igloo and disposable cups. This facility plans to provide Incidental Medical Services – IMS and has submitted a Plan of Operation. Applicant is requesting a playground waiver due to space limitation because the Preschool playground is only big enough to accommodate 83 children at one time. Proper supervision and placement of staff on the play yard was discussed to ensure the safety of children at all times. A fire clearance was granted on 06/14/19 for 160 Preschoolers.

A playground waiver must be submitted and approved prior to granting a license.
