25 | On July 8, 2022, at 2:20 p.m., Licensing Program Analyst (LPA), Gloria Gonzalez conducted an unannounced Annual Required Inspection and met with the Licensee, Raquel Campos. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Five (5) daycare children and one (1) staff were present in the facility during this inspection. This facility is a two story, 6 bedroom, 4 bathroom house. Bedroom #1 and bathroom #1 are downstairs and has been part of the home as seen on the previous facility sketch that was received in 8/31/12, but not included in the report at that time. This room and bathroom are divided by a wall in the kitchen. Bathroom #2 is downstairs and used for daycare which was also included in the facility sketch dated 12/5/13 and approved and documented on LIC812 dated 12/10/13. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: dining room, living room, kitchen, bathroom #2, nap area, and backyard. Off limits areas are downstairs bedroom #1, bathroom #1, garage and all of upstairs which is bedrooms #2, #3, $4, #5, #6, bathroom #3, #4, and are inaccessible through use of door locks and a safety gate. Hours of operation are: Monday - Friday from 8:00 am - 10:00 pm.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. Hazardous items were made inaccessible to children during the inspection. The licensee has toys, play equipment and materials available. The home has a fenced backyard available for outdoor activities at time of inspection. The upstairs is not used and is gated off at the bottom of the stairway and the applicant understands the gate must be in place when children under five years are present during day care hours. No bodies of water observed on the premises during the inspection. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. |
