On 05/30/2023 at 02:40 AM, Licensing Program Analyst (LPA) David Miller and Licensing Program Manager (LPM) Tulam Vu, conducted an unannounced Annual Required 1 year inspection and met with the licensee Carmen Anzaldo. LPA disclosed the purpose of the inspection and was granted facility entry by the Licensee. The licensee, two (2) infants, and three (3) preschool children (ages 2-4) were present in the facility during this inspection. ID16280 and ID15268 from Language Link provided translation from Spanish. This single story, 4 bedrooms, 3-bathroom home was inspected. The following areas are used for day-care: living room, dining room, daycare bathroom, daycare bedroom, family room, kitchen, nook, and east patio. Off limits areas include: bedroom 1, bedroom 2, bedroom 3, bathroom 1, bathroom 2, garage and are made inaccessible to day care children through the use of doorknob covers and safety gates.

The fire extinguisher and smoke/carbon monoxide detector met requirements. The licensee has available toys, play equipment and materials. Children play outside in the fenced backyard. Licensee was reminded that continuous supervision is to be given to children whenever engaged in outdoor activities. No bodies of water were observed on the premises during the inspection. Licensee stated there are no weapons in the home and LPA did not observe any firearms at the time of this inspection.

Licensee’s First Aid and CPR certifications expire in 04/2025. Licensee has required immunizations. Licensee completed the mandated reporter training on 01/26/2022. The facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 05/02/2023 (fire) and 05/03/2023 (earthquake). .

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reminded Licensee of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms, and ensure that all adults residing or working in the home have criminal background clearances or exemptions. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare.

Incidental Medical services (IMS) policy was discussed and reviewed. Licensee stated that she currently has one child with IMS. LPA inspected and medication is currently being stored properly. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. No deficiencies cited.

A copy of the report and appeal rights (LIC 9058) was provided to the applicant and notice of site visit (LIC9213) was given to Licensee and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee Carmen Anzolda.
