25 | On September 2, 2021, at 1:00 p.m., Licensing Program Analyst (LPA), Gloria Gonzalez conducted an unannounced Annual Required Inspection. LPA disclosed the purpose of today's inspection which is to also inspect the coverted garage to be used for childcare use and was granted a tour into the facility by the Licensee Alma Ruiz. Four (4) children and two (2) staff were present in the facility during this inspection. This facility is a two story, 4 bedroom, 3 bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: Living room, kitchen, dining room, bathroom 1, bedroom upstairs room #1 and upstairs bathroom #2, family room, daycare area 1, new daycare room 2, and backyard. Off limits areas are upstairs, bedroom #2, #3, #4, laundry room, bathroom #3 and are inaccessible through use of a safety gate and door knob covers. The fire clearance for the new day care room (converted garage) was received on 9/2/21. New daycare room was inspected and cleared to use for daycare use. Hours of operation are: 6:00 am to 10:00 pm and 5:00 pm to 5:00 am depending on parents schedules.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. Hazardous items were made inaccessible to children during the inspection. The licensee has toys, play equipment and materials available. The home has a fenced backyard available for outdoor activities. Bedroom 1 and bathroom 2 is being used upstairs as needed for night shift use and is gated off at the bottom of the stairway and the applicant understands the gate must be in place when children under five years are present during day care hours. No bodies of water observed on the premises during the inspection. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances. Licensee’s First Aid and CPR certifications expire on 2/2023. Licensee has required immunizations. Licensee completed Mandated Reporter Training on 3/11/20. Facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 8/3/21. There is one play yard for each infant who is unable to climb out of the play yard. Play yards are free from all loose articles and objects. Licensee physically checks on sleeping infants up to 24 months of age every 15 minutes. An Individual Infant Sleeping Plan [LIC 9227 (3/20)] is maintained for each infant up to 12 months of age. Licensee places infants up to 12 months of age on their backs for sleeping. |
