25 | On August 26, 2021, at 10:00 a.m., Licensing Program Analyst (LPA), Gloria Gonzalez conducted an unannounced Annual Required Inspection and met with the Licensee, Guadalupe Vizcarra Meza. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Seven (7) children and two (2) staff were present in the facility during this inspection. This facility is a two story, 4 bedroom, 3 bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: kitchen, living room, dining room, bathroom 1, laundry room, and backyard. Off limits areas are closet by staircase, and all of upstairs (bedroom 2,3,4, bathroom 2,3) and are inaccessible through use of a door knob cover and a safety gate. Hours of operation are: Monday through Sunday from 4:00am to 12:00 am.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. Hazardous items were made inaccessible to children during the inspection. The licensee has toys, play equipment and materials available. The home has a fenced backyard available for outdoor activities. There is construction being worked on outside adding a room that will not be used for children. Licensee states she will supervise children during outside activities at all times. Licensee states due to hot weather backyard will be used at a minimum. The upstairs is not used and is gated off at the bottom of the stairway and the applicant understands the gate must be in place when children under five years are present during day care hours. No bodies of water observed on the premises during the inspection. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances. Licensee’s First Aid and CPR certifications expire on 8/2023. Licensee has required immunizations. Licensee completed Mandated Reporter Training on 8/10/20. Facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 06/4/2021. Provider was advised when an infant is in care, there is one play yard for each infant who is unable to climb out of the crib or play yard. Cribs or play yards are free from all loose articles and objects. The provider physically checks on sleeping infants up to 24 months of age every 15 minutes. Licensee advised if there is an infant in care, an Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be maintained for each infant up to 12 months of age. Provider was advised to place infants up to 12 months of age on their backs for sleeping. |
