25 | On 05/09/2023, Licensing Program Analyst (LPA) Candis Rodriguez conducted a complaint inspection at facility on the preschool license. LPA met with Site Supervisor Crystal Mallard and Director Sarah Vinson.
During investigation of the complaint, it was revealed that there are two staff members employed by Canyon Hills Church that have an office in the school age/Fort Faith building which is licensed to the school age child care. It was revealed that Staff #1 and Staff #2 are not finger print cleared through the Department, and they are not associated to either the preschool or school age licenses. It was revealed through interview that Staff #1 and Staff #2 are present during operating hours, and may interact with children in passing under both school age and preschool licenses, as well as assisting facility with electronics issues and assisting in Chapel time weekly with school age and preschool age children.
Due to Staff #1 and Staff #2 being employed by the Licensee, and being present in the facility during operating hours Tuesday through Friday, they are both required to be finger print cleared through the Department. LPA advised Director Staff #1 and Staff #2 should not be present during operating hours until they have received their clearances. LPA also discussed the other documentation requirements for employees to keep in their files for review by the Department.
Per California Code of Regulations Title 22, Division 12, a type A deficiency has been cited on the school age license (see report dated 05/09/2023 on school age license, facility number 153808222 for more information). LPA issued a Civil Penalty to facility in the amount of $1,000. Licensee was provided a copy of their appeal rights.
LPA informed Director that this report documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
