On 3/29/2023 at 9:48am, Licensing Program Analyst (LPA) Andrea Pittman arrived at the facility to conduct an Annual Random Inspection. LPA was met by Director Jessica Krall and was allowed access to the facility. Director Krall stated that she was covering as the Teacher in Classroom D and LPA toured the facility in accordance with the facility sketch. During the inspection, LPA observed 5 children and one staff with two student interns providing care and supervision. Facility operates Monday through Friday from 7:45am to 5:00pm.

Facility consists of a total of 21 rooms and/or areas and an outdoor play area separated by fencing for each license. The center operates currently using three classrooms: Room A, Room C, Room D, with Room E1, E2, and O currently not in use, one classroom, four offices, one kitchen, one pantry, one staff restroom, two children’s restrooms located in Rooms C and D, one changeroom with a children’s restroom in Room A, one lobby restroom, one laundry area, and an outdoor play area with fencing separating the children by license.

Child care occurs on the infant license in Room A, the changeroom with a children’s restroom, and the infant’s outdoor play area.

Indoor Activity Space:
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. LPA observed sign-in sheets completed by each parent/guardian per requirements in Room A. LPA observed individual storage for children's belongings and individualized bedding for each child in care. LPA observed children have access to water via filtered water in large drinking containers dispensed in individual, one-time use disposable cups. An isolation area was located in the Director’s Office with a separate restroom located nearby, the lobby restroom. LPA observed children are provided napping and age-appropriate mats were observed with individual bedding materials.

LPA observed age-appropriate sinks and toilets and they were inspected for availability and good repair. Toilets flushed properly; toilet and sinks are reachable by the children. LPA observed a changing table within reach of the sink with a trash can with a fitted lid in the restroom area. At 10:51am, LPA observed cleaning supplies on the changing room sink accessible to children in care which poses a potential risk to children in care. Staff 1 addressed this inefficiency by directing Staff 2 to place the cleaning supplies under the restroom sink which was secured with a safety latch making the cleaning supplies inaccessible to children in care. This is a Type B Violation. The restroom has adequate toilet paper, hand soap, and paper towels available. LPA observed the restroom to be clean, restocked with toiletries, and sanitized by on-site staff.

Per Director, smoke detectors are linked to a system with the college and monitored by the college. The carbon monoxide detector was tested and found to be operable. Fire extinguishers were observed and in operable condition. Smoke and carbon monoxide detectors are operating according to Fire Marshall standards. First Aid Kit shall be complete with the following supplies including tweezers, scissors, gauze, bandages, cleansing pad/solution, thermometer, and a first aid manual. LPA observed a First Aid Kit that was complete. Staff 1 ensured the bag to be inaccessible to children in care by placing the bag in a cabinet with a safety latch after outdoor play. At 10:38am, LPA observed a trash can with a tight-fitted lid in the restroom only. LPA observed trash cans with solid wastes without tight fitting lids in the outdoor play area and in Room A, Staff 1 addressed the inefficiency by moving the trash cans without lids to areas inaccessible to children in care. This is a Technical Violation.
Emergency Drills are conducted at least every six months and the last drills were conducted and documented on 2/3/2023 at 9:00am for both the Fire Drill and the Emergency/Earthquake Drill. The fire extinguisher (4A60BC) is reading in green and meets the Fire Marshal Codes and Standards (last serviced 8/8/2022). Menus are posted in the Reception/Waiting area at least a week in advanced, dated, and in sight of parents to review. The facility was observed to have warm water and temperature falls within requirements. The Room temperatures throughout the facility was observed to be at or above 68 degrees F but below 85 degrees F. The kitchen was observed to be inaccessible to children in care. LPA observed a small refrigerator in the classroom, the food inside was found to be in good condition, labeled, and dated. Daily activity schedules are posted in the classroom showing meal/snack times, nap time (if given), and specific activities. Food allergies are noted and kept on a list in the cupboard and in the kitchen. Per Staff 3, children with allergies have their food prepared separately. There is no outside food allowed and the center provides breakfast, lunch, and snacks.

Outdoor Activity Space:
The outdoor play area was inspected and potential hazards to children in care were observed. Equipment was inspected for safety, cushioning material, good repair, and age appropriateness. At 10:26am, LPA observed there were four children’s chairs that had paint peeled off and the metal showing underneath was rusted. Staff 1 addressed this inefficiency by placing the rusted children’s chairs in an inaccessible area to children in care. This is a Technical Violation. Climbing structures and other play equipment were found to be securely anchored with adequate resilient cushioning material underneath and around the perimeter. Children are provided with filtered water via a container and disposable, one-time use cups during outdoor play. There is adequate shaded area for rest. The playground is well-fenced, all around and no bodies of water were observed in the outdoor play area.

Facility maintains an adequate teacher-child ratio. Care and supervision were evaluated and determined basic needs of children are appropriate and are being met. The parent board was reviewed and has all the required forms posted accessible to parents.

Records/Documentation:
Children's records were reviewed for completeness. Health History, Emergency contact and Medical Exams; Immunization Records are all in the children's file.
The facility roster was up to date and all staff have been fingerprinted and association to the designated license number. Personnel records were reviewed for completeness. Director and teachers are currently certified in Pediatric CPR/First Aid, expires 1/7/2025. Child Care Providers (AB1207) Mandated Reporter training certificate is dated 1/11/2022.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with the Facility Representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Facility Representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following Incidental Medical Services (IMS) were discussed.


This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Upon hired date all staff are required to be immunized and show proof of immunization records according to Title 22 regulations.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from date of employment to complete training as required. Director is aware self and all staff are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hot-line at (800) 540-4000.
