25 | On 11/21/19 at 9:37 AM Licensing Program Analysts (LPA) Ortega met with Site Director, Megan Hall-Silveria for an unannounced Case Management Self reported incident inspection that occurred at the facility on 11/07/19. Upon entry to the facility, LPA Ortega verified a census of 74 children in care. LPA disclosed the purpose of the inspection and was granted entry by the Site Director.
Description of the incident:
On Thursday, November 07, 2019 at approximately 9:05 AM Staff reported that Child #1 exited the classroom (Room 6) and wondered onto the yard unnoticed. Child was found on the playground walking alone. Child did not sustain any injuries. Parent was notified of the incident in a timely manner.
The purpose of the inspection is to follow up with the facility regarding the incident dated 11/7/19.
Based on LPA's observation, interviews conducted with staff, documents gathered and record reviews, the preponderance of evidence standard has been met, therefore the above incident is found SUBSTANTIATED. Day care child#1 was found in the yard alone for approximately 5 minutes due to lack of supervision. Interviews conducted with staff indicated child was found wondering around the yard away from the child's designated classroom. According to the evidence gathered the perimeter gate was left opened permitting access to child#1 to wonder off unsupervised from classroom(room 6) to the play yard unnoticed. California Code of Regulations, Title 22, Division 12 and/or Health and Safety Code, are being cited today. Type "B" deficiency under lack of supervision will be cited today 11/21/19.
Exit interview conducted, a copy of this report, notice of site visit and appeal rights were provided to Megan Hall-Silveria, Site Director. |
