Licensing Program Analysts (LPA's) Justin Dorsey and Joselito Del Mundo spoke with Director, Meghan Hall-Silveira, for the purpose of conducting a follow-up on a Case Management Unusual Incident Report (UIR) from the facility. This UIR was received by Licensing on 10/03/22.

Description of incident: On 09/30/22 the facilities maintenance team discovered mold under the rubber wall trim in the centers food storage room. According to the Incident Report all foods are factory sealed and safe for consumption. The maintenance team removed shelving and foods and sealed the room for specialized inspection and mold abatement.

During this visit, LPA's received documents pertinent to the incident and interviewed Director Meghan Hall-Silveira. According to the Director the mold was caused by a leak behind the wall in the storage room from a sink in the laundry room. The Director showed LPA's the food storage area, laundry room and kitchen. LPA Dorsey observed that the room used to store the facilities food to be sealed by plastic wrap and tape. Per Director only the maintenance team has access to the affected area. LPA Dorsey observed the center now stores food that does not need to be refrigerated on shelves in the center hallways. LPA Dorsey observed the food to be sealed. Per Director a bid was accepted for the removal of the mold and the facility is in the process of getting approval from the Child Adult Care Food Program so that the mold can be removed.



Notice of Site Visit shall be posted for for thirty (30) consecutive days. Failure to maintain posting as required will result in a $100 civil penalty.

An exit interview was conducted with and a copy of this report has been signed by and provided to Director Meghan Hall-Silveira, Notice of Site Visit and Appeal Rights were given.
