On 8/13/2021 Licensing Program Analyst (LPA) Brannon, conducted an unannounced Annual Required Inspection for the school age license. LPA met with Director, Monica Gutierrez, and two board members, #1 and #2. LPA toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday, 6:30 AM to 6:00 PM.

Upon arriving at the facility, LPA observed school age children playing in the front of the facility lobby doors. There is grass, tables but no fencing. LPA inquired as to why the school age children are outside playing in an unfenced area. Per director, she was told that the area is licensed by previous director. The school children use the unfenced area instead of using the preschool area when the preschool children are outside playing. LPA responded that in order for an outside area to be licensed, fencing that meets Title 22 requirements would be installed. Children’s files do have a field trip permission slip signed by parent or legal representative.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. LPA reviewed the requirement to keep open bags of food in zip lock or plastic containers. LPA observed one open bag of cereal. Kitchen is in the process of being enlarged.

The process is not interfering with kitchen staff providing AM and PM snacks to children. Currently, the parents are providing lunch. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. LPA was informed that children are drinking from the water fountains for inside and outside drinking water source. LPA informed director that is recommended to have staff sanitize the drinking fountains after each use to help mitigate the spread of COVID. Instead, licensee can utilize a water pitcher or igloo with a disposable cup dispenser and disposable cups to provide uncontaminated drinking water for the children in care. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director, Monica Gutierrez or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 14 school age children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Menus are posted at least one week in advance where an authorized representative can view them.

During today's inspection, LPA was informed that staff and parishioners were using the school age bathroom. A door leading from the lobby into the licensed school age bathrooms area is unlocked. This allows individuals to enter the school age area. LPA informed director and the two board members that the door leading into the licensed area will need to be locked. The other doors leading into the area are locked. LPA was informed that staff also utilize the school age bathrooms. LPA instructed director, Monica Gutierrez, to submit a waiver for the school age children bathroom for approval. The waiver is required to be received by Fresno Community Care Licensing office by 8/31/21. Today was the school age program last day. The school age program will reopen on 8/18/21. Per director, this practice has been on-going for years. LPA reviewed section 101239 Fixtures, Furniture, Equipment and Supplies with director and provided a copy of regulations. Staff does have access to adult bathrooms in the building next door. No adults should be using bathrooms licensed for school age and preschool children. These bathrooms are counted towards the licensed capacity.

Incidental Medical Services (IMS) in place, however, there are no children requiring IMS. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
