On Friday, February 14, 2025, Licensing Program Analyst (LPA) Paul Garcia conducted a complaint inspection. LPA Garcia met with Director Cheri Mosley then explained that the purpose of the inspection was to deliver the findings for the above allegations. LPA toured the facility and took a censes.

During this investigation, LPA Garcia interviewed Administrators, rank and file staff members and witnesses. This investigation revealed through testimony obtained, children that were determined to be ill, were not isolated away from other children in the Terrific Tigers classroom placing them at risk and based on information received during an interview with Director Cheri Mosley, the facility at the time had no dedicated isolation area for children with illnesses as required per conditions of licensure.

This agency determined that the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 12, Chapter 1, a deficiency will be cited (see 9099-D). An exit interview was conducted with the Director, Cheri Mosley.

On December 18, 2024, Director, Cheri Mosley designated a dedicated isolation area for children with Illness located in a room next to the front desk where children will solely be supervised by a dedicated staff member adhering to Title 22 101226.2 Isolation for Illness. Director, Cheri agrees to also conduct an

determined that staff allowed day care children with symptoms of illness to attend the facility and staff did not isolate ill day care children from well children. This poses a potential risk to the health, safety, or personal rights of children in care.

informal training/discussion with her subordinate staff on the importance of quickly isolating ill children away from well children to assist in unnecessary exposure to other children in care.
Director Cheri agrees to provide LPA with an outline of what was discussed with signatures of staff who were meet with by March, 5, 2025.
