25 | On 10/11/2022, Licensing Program Analyst, (LPA) Theresa Marquez conducted a Case Management inspection and met with Director, Cheri Mosley. The purpose of this inspection is in regards to a self reported incident that occurred on 9/12/2022.
On 9/12/2022, Child #1 was playing around the sensory Lego table. Child #1 stood on top of a chair, jumped off the chair, and hit his left pinky finger on the corner of a second sensory Lego table. Teacher A witnessed child #1 jump off the chair. The child continued to play and it wasn't until the child went inside he was observed holding his left hand. Teacher B inspected the child's finger and did not observe any visual injuries.
When child #1 was picked up from day-care by his father, the child began to cry while being put into their vehicle. The child was taken to his primary Dr. and x-rays revealed the child's pinky finger was fractured. A stint was placed on the child's finger on 9/14/22. Child #1 remained at home and returned to the center on 9/19/2022.
LPA Marquez discussed with Mosley supervision at the time of the incident. LPA Marquez recommended to Mosley when a child is injured it is staff responsibility to monitor the child instead of waiting for the child to complain of pain.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations no deficiencies were observed today.
Exit interview was conducted with Director Cheri Mosley. LPA provided Mosley with a copy of the Facility Evaluation Report (LIC 809), and the Notice of Site Visit form (LIC 9213). The LIC 9213 is required to be posted for 30 days. |
