On 07/07/2025 Licensing Program Analyst Isabel Ortega met with facility Director, Candace Monette and conducted an Annual-Random Inspection. LPA toured and inspected the facility in accordance with the facility sketch. Facility is licensed for a total of six classrooms for preschool care ages 2 to 6 years with a toddler component. At time of entry LPA observed 29 children in care and 10 Staff providing care and supervision.

Facility operates Monday through Friday from 6:30 a.m. to 6:00 p.m.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. LPA observed individual storage for children’s belongings labeled with child’s name. The classroom consists of filtered water with individual water bottles for children. (LPA observed water filtered water jugs and individual bottles in each classroom). Facility tested water for lead on 12/30/2022, results were under the 5.5 ppb allowable.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Facility provides children with scheduled breakfast, lunch and one afternoon snack. Facility has extra snacks available when needed. Food was inspected, and are properly labeled, stored within expiration date.

Age-appropriate sinks and toilets were inspected for availability and good repair. Toilets flush properly; toilet and sinks are reachable by the children. Each restroom has adequate toilet paper, hand soap and paper towels available. Restrooms are cleaned, restocked of toiletries, sanitized and disinfected by staff and janitor daily.

An isolation area was inspected, which takes place in the main office. Children are provided with individual mats. The blankets are provided by parents and stay on site for weekly wash and sheets are provided by facility and washed weekly. Facility has an Emergency Kit in the classroom and a thermometer.

First Aid supplies, emergency food, smoke detectors, carbon monoxide were observed and in operable condition. The fire extinguisher is serviced yearly; LPA observed a white tag with serviced date on 4/08/2025. Trash cans with tight lids were observed. Cleaning supplies are out of reach of children, during inspection Facility removed chemicals from the kitchen area and relocated them to an off-limits area.

Emergency Drills are conducted every six months, last drill was conducted and documented on 05/27/2025 at 9:15 a.m. Food allergies are noted in the classroom and are posted in the food preparation area.

The outdoor play area was inspected and observed to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, good repair, and age appropriateness. During indoor and outdoor play children are provided with non-contaminated water, which was sampled, and results were under 5.5 ppb on 12/30/2022. There is adequate shade area for rest. The playground is well fenced all around, and no bodies of water observed in the outdoor play area. Small wadding blue pools were observed empty. Facility is aware to empty each wading pool after each use.

During the inspection LPA observed adequate teacher child ratio in each classroom. Care and supervision were evaluated and determine to meet the basic needs of children. A review of the sign in/out sheets were conducted to verify the current census of children. The parent board was reviewed and has all the required forms posted accessible to parents.

Children's records were reviewed for completeness. Health History, Emergency contact and Medical Exams, Immunization Records are all in the children's file. The facility roster was up to date and all staff have been fingerprinted and association to the designated license number.

The Director is currently certified in Pediatric CPR/First Aid from American Red Cross which does not expires until 04/16/2027.

The following Incidental Medical Services (IMS) were discussed.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. All staff are required to take the Mandated Reporter training every 2 years at www.mandatedreporterca.com. Childcare Provider Mandated Reporter (general/Child Care Provider/Exam) is dated 3/4/2024.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com. Director is aware all staff are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hotline at (800) 540-4000.
