On 09/08/2022, An unannounced complaint inspection was conducted today by Licensing Program Analyst (LPA), Jose Penate. LPA met with Site Supervisor, Leticia Lopez. LPA advised the purpose of today's inspection is to close the complaint investigation. During investigation, LPA interviewed staff, interviewed children, reviewed facility records, and observations conducted.

With the allegation of, day care child was hit by another child while in care, was investigated and based off interviews, and records review conducted this incident did occur at the facility.

LPA verified with staff at the facility that a child with some difficulties does act out by throwing items at staff and children. All children are being supervised and cared for but the child lashes out randomly when least expected and unfortunately children and staff do have injuries at times from the child acting up.

This agency has investigated the complaint. After interviews, records review and LPA observance this agency has investigated the above allegation and has determined that a child in care has had multiple incidents of throwing toys, biting and hitting children in care, the preponderance of evidence standard has been met; therefore the above allegation is found to be SUBSTANTIATED.

Per California Code of Regulations, Title 22, Division 12, deficiency cited (see LIC 9099-D).
