25 | On 9/29/22, Licensing Program Analyst (LPA) Jessika conducted a Case Management inspection. LPA Thompson met with licensee, Luz Urena . Also present were Staff #1 (S1)and four day-care children. Licensee is primarily Spanish speaking; therefore, S1 provided translations services. The purpose of this inspection was to address a deficiency discovered at the facility.
During today's inspection, LPA found that the the licensee has not been updating her facility's Children's Roster (LIC9040) accordingly. LPA reviewed facility records and found that all four children present today were not listed on the Children's Roster as required.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited: (see next page, LIC809D). Licensee was provided a copy of appeal rights.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee.
