25 | On October 06, 2022, Licensing Program Analysts (LPAs) Isabel Ortega and Anna Villa met with facility Supervisor Francis Vega to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self reported unusual incident report (UIR) submitted to the Department that occurred on August 23, 2022. The unusual incident report was regarding Child #1 and Child #2 were inappropriately handled by Staff #4.
Upon arrival, there were 36 children observed in care and 8 staff.
During this inspection LPAs conducted interviews that disclosed Staff inappropriately handling children #1 and child #2. In addition, LPAs completed a safety inspection of the facility. In addition, during the inspection, LPAs obtained copies of documentation related to the case management incident. LPA's have concluded this incident is a personal rights violation of Child #1 and Child #2, cited today under Section 101223(a)(1) Personal Rights. Each child receiving Child Care Services shall be accorded dignity in his/her personal relationships with staff, residents and other persons. Due to information received, incident is rendered a Type B violation. A notice of site visit was provided and requested to be posted for 30 days.An exit interview was conducted, a copy of this report, notice of site visit and appeal rights were provided to facility.
