Licensing Program Analyst (LPA) Gloria Reyes made an unannounced Annual/Random inspection. LPA met with Director of Child Development, Michael Bledsoe and Site Supervisor, Celina Fernandez. This is an AM/PM half day program which operates on a traditional school year schedule. The morning program operates from 7:45 AM to 10:45 AM and the afternoon program operates from 11:45 AM to 2:45 PM. There are five classrooms, however only three classrooms are in operation. There are no bodies of water. Firearms/weapons are not allowed or stored on premises. All children are under supervision, including visual supervision, of a teacher at all times. The Department has inspection authority. There is a ratio of one teacher supervising no more than 12 children in attendance. Disinfectants, cleaning solutions, poisons and other dangerous items shall be inaccessible to children. Storage areas for poisons are locked. All materials and surfaces accessible to children are toxic free. Medications are in a safe place inaccessible to children when available. All toilets and hand washing facilities are in safe and sanitary operating conditions. All floors are clean and safe. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Breakfast and lunch is prepared in the District Central Kitchen and prepped in the Albany Park Elementary School cafeteria. Meals are served in the school cafeteria. The day care children are escorted and supervised to the school cafeteria for meals. No snacks are provided. Uncontaminated drinking water is available both indoors and outdoors. Menus are posted at least one week in advance, where an authorized representative can view them, are dated and kept on file for 30 days, and are available on request. LPA verified that the Mandated Child Abuse Reporter training required by staff has been completed. LPA verified that the required immunizations have been completed by staff. (see next page)

Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Playground equipment is in good condition, free of sharp, loose, or pointed parts and with adequate cushioning via wood chips. Outdoor activity space surface is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings, and slides have cushioning material to absorb falls via wood chips. CCL shall notify a licensee to immediately terminate the employment of, or to remove/bar any person with specified convictions or for other reasons. The licensee shall comply with the notice. Before working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have a clearance or exemption and have been associated to the facility. As a condition of employment, Delano Union School District has all employees fingerprinted and requires a health screening with TB clearance. Staff records contain appropriate, documentation of education credits. At least one person trained in Pediatric CPR and Pediatric First-Aid is present when children are at the facility or at off-site activities. The person, who signs the child in/out, is responsible for the child, uses their full legal signature and records the time of day. Child's admission agreement is available for review. This facility provides Incidental Medical Services – IMS. However, no children are currently receiving medications at this time in the AM session. An exit interview conducted with Director of Child Development, Michael Bledsoe and a copy of this report was provided and discussed. A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days.
