25 | A case management inspection was conducted today by Licensing Program Analyst (LPA), Caroline Harris. LPA met with Director, Janae Hansen to discuss incident which occurred on 6/5/19. A complete file review was conducted prior to visit. LPA toured facility and observed upstairs classrooms and bathroom. Census was taken. LPA interviewed staff and observed area in which incident occurred. Child's sign in/out sheets, staff sign in/out sheets, emergency protocol and school activities were gathered.
On 6/5/19, at approximately 12:00 p.m., a child was found in the hallway unsupervised after going to the bathroom. The child was talking to some other kids in the hallway and the parent of those kids accompanied the child back to the classroom. When returning to the classroom, the child complained of their tummy hurting. At approximately 2:15 p.m., teacher, Ms. Crossley contacted the child's parent as the child stated that they were dizzy and the child was acting strangely. The child’s parent picked up the child at approximately 2:45 p.m., and transported the child to the doctor. It was later determined that the child had alcohol in their system and the child stated it was from ingesting hand sanitizer. During todays inspection, the LPA observed hand sanitizer dispensers in the upstairs hallway and was also notified that hand sanitizer was previously in the classroom on the counter, however it has all been removed. The licensee reported the incident to Community Care Licensing.
California Code of Regulations, Title 22, Division 12, Chapter (1/3), are being cited on the attached LIC 9099D. An exit interview was conducted with Janae Hansen. An informal/non compliance conference will be scheduled. A printed copy of this report as well as appeal rights, Parent Notification Requirements and LIC 9224 was provided to Janae Hansen at the conclusion of the visit.
* Any Licensing reports indicating a Type A deficiency shall be posted immediately and for the next 30 days and copies provided of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months 1596.8595(c). Health & Safety Section 1596.859(a) shall be cited and a civil penalty of $100.00 for failure to provide copies to parents/guardians of children in care and newly enrolled children, and for failure to maintain written verification of receipt of licensing reports indicating a Type A violation (LIC 9224).
