On November 3, 2023 Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced case management visit and met with Director Lori Rogowski. LPA McWilliams toured the facility and a census was taken. LPA McWilliams informed Director Rogowski the purpose of the inspection was due to a unusual incident report that occurred on October 31, 2023 late in the afternoon. It is documented Director Rowgowski attempted to report on November 1, 2023 but was unable to. Director Rogowski self-reported to the Department on November 2, 2023.

It is reported that child 1 (C1) was left alone on the playground after PM recess for approximately between 5-10 minutes. C1 was observed outside by themselves by another parent coming to pick up their child. It is reported that the staff member (S1) did not see C1 behind play kitchen set.

Director Rogowski has removed all kitchen play sets outside, has implemented another safety check system with transitioning children from the playground to inside. Director Rogowski also went over the facilities current transitioning plan with all preschool staff ensuring that all of the children are accounted for during transition periods. S1 was immediately let go for failing to keep child safe.

Exit interview conducted and report was reviewed with Director Rowgowski . Appeal rights were provided.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).

Upon receipt of a Type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Child Care Parent Notification Requirements and a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports was given to Licensee.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Director states that she has implemented an additional safety measure of checking children between transitions, Director has removed all toy kitchens from the play ground and has gone over the facilities policies on counting children in transitions.

This requirement was not met by self-report to the department that C1 was left outside unsupervised for 5-10 minutes. This poses an immediate health safety or personal right to a child in care.
