25 | An unannounced case management - incident inspection is conducted this date by Licensing Program Analyst (LPA) Gloria Reyes. An Unusual Incident Report was submitted to Fresno Community Care Licensing. LPA met with Site Supervisor, Rosalva Banuelos regarding an incident which occurred at the facility on 11/19/19, at 9:35 AM during transition time from outdoor play to indoor play. Two day care children were on the play structure at the slide area. Child #1, wanted to stay outside. Child #2, asked child #1 to slide down, but child #1 refused. Child #2, reacted by pushing child #1 and child #1, fell on top of child #2. Both children stood up. Immediately child #2, pushed child #1 forward and child #2 fell on top of child #1, then child #2 bit child #1 on the upper right side of his/her back. Staff #1, was assisting another child at the climber near child #1 and child #2, who heard and observed the incident. Staff #1, stated that this incident was the first time child #2 bit another child and this is not the child's usual behavior. Staff #1 administered first aid and another staff contacted the parent. The facility advised the parent to seek medical treatment for the bite. The parent later informed the facility that the child was taken to the doctor. The doctor prescribed medication, released the child and returned to the facility the next day. The child is doing fine.
On the day of the incident there were 3 staff and 21 children present. Each staff were assigned 8 children. LPA interviewed staff #1, who observed the incident. LPA also spoke with the Site Supervisor Rosalva. LPA observed the area where the incident occurred and LPA did not observe any hazards. Miss. Rosalva explained that the facility is working with the School District and an Action Plan has been developed to assist child #2. The School Psychologist will also be provide assistants to the facility. The facility also hired a Behavioral Health Aid to assists children with challenging behaviors at the facility. LPA reviewed the child's file and verified that the plan is being followed. This appears to be an isolated incident and staff took appropriate measures to address the child's needs. Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiencies cited.
An exit interview conducted with Site Supervisor, Rosalva Banuelos and a copy of this report was provided and discussed. A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days. |
