25 | Licensing Program Analyst (LPA) Gloria Reyes conducted an unannounced case management incident inspection to follow up on an unusual incident that occurred at the facility on 12/20/19.
The incident was reported via telephone and the Unusual Incident Report (LIC 624) was received by Fresno Community Care Licensing on 12/23/19. LPA met with Site Supervisor, Yuliana Casas to discuss the incident. A tour of the facility was conducted. The children were napping during the visit.
The incident occurred around 11:09 AM, when the class were transitioning from outside to inside the classroom. Staff #1, was bringing two children inside the classroom. Staff #1, was holding the hands of child #1 and child #2. Child #1, fell and Staff #1 reached down to pick up the child. Child #2, had placed his/her hand between the door frame and when the door closed, Child #2's left pinky was bleeding and partial removed. Staff #1, administered first aid while another staff contacted the parent. The parent picked up the child and was advised to take the child to the doctor. The child is doing fine and has returned to day care center.
The Site Supervisor conducted training with her staff. Staff were asked to check the doors before closing them to ensure that this type of injury is not repeated. LPA asked Site Supervisor Yuliana to purchase a "Door Pinch Guard" to prevent future injuries within the next week. Site Supervisor Yuliana to send a photo to licensing. This appears to be an isolated incident and the director took appropriate measures to address the child's injury.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiencies cited.
An exit interview conducted with Director, Yuliana Casas. A copy of this report was provided and discussed. A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days. |
